Lapeer CMH employs a hospital liaison who provides
services to Lapeer County residents admitted to the
behavioral unit of a hospital or a psychiatric facility
for inpatient mental health treatment. The liaison
is available before, during, and after a hospital
admission to insure continuity of care. The
liaison can assist the person who is hospitalized with
meeting their needs, which may include establishing
outpatient treatment and providing information regarding
other resources when the individual is discharged from
the hospital. Aftercare mental health services are
coordinated with the consumer’s primary case holder at
CMH and an outpatient appointment is arranged with both
the case holder and a staff physician. The goal is
to have a discharge plan in place that involves the
consumer's input, is manageable for the consumer,
encourages follow through, and improves recovery
outcomes.
· Admission assistance including
assistance in obtaining a court ordered petition
when a person is in need of
psychiatric hospitalization
· Assessment of the possible need for
inpatient treatment by the Triage staff
· Arranging for an ambulance or police
escort if necessary
· Regular contact with out of county
hospitals to promote coordination of care
· Frequent visits to Lapeer Regional
Medical Center Behavioral Health unit for staff
team meetings
· Discharge planning includes
providing information for locating needed
resources.
· Setting up a follow-up appointment
with Lapeer CMH within 7 days of discharge
· Coordination with CMH physician and
scheduling an appointment within 30 days of
discharge
· Reviewing options with each person
to find the least restrictive environment for
optimum therapeutic benefit and recovery
